Student Registration Form
Students wishing to attend as a presenter or observer must be nominated by a teacher from their school. After obtaining a nomination, students then fill out the Student Registration Form. The form is fillable, however, it must be printed for Guardian, Nominating Teacher, and Principal signatures. The completed form can be mailed to the address at the bottom of this page.
Nominating Teacher Registration Form
Teachers wishing to nominate students as a presenter or observer must complete the Nominating Teacher Registration Form. The form is fillable, however, it must be printed for Nominating Teacher and Principal signatures. The completed form can be mailed to the address at the bottom of this page.
Nominating Teachers are expected to attend the symposium and may register on this form, however, an alternate Chaperone can be designated if necessary. Nominating Teachers are still responsible for ensuring students have completed all registration and paper submission forms.
Chaperone Registration Form
Non-nominating Teachers and Parents are welcome to attend. If the non-nominating Teacher or Parent is acting in the capacity of an official Chaperone, the cost of attending may be covered. IJSHS will cover the cost of food, accommodations, and in-symposium transportation of one Chaperone per every four students from a school. Special considerations may be made on a case by case basis. Chaperones must complete the Chaperone Registration Form. The form is fillable, however, it must be printed for Nominating Teacher and Principal signatures. The completed form can be mailed to the address at the bottom of this page.
Hotel Room Assignment Form
Nominating teachers are requested to complete the Hotel Room Assignment Form. If the form is not submitted, hotel rooms will be assigned. IJSHS will attempt to group schools together (with room assignments based on gender), but assignments cannot be guaranteed.
